Declaration (please read and accept by checking the box and clicking next)

As a Volunteer/Member with Maranoa Wildlife Caring and Education Centre (MWCEC, I declare that :

1. I am over 18 years of age.
Have the authority to sign on behalf of the business / company listed on this application form
2. Understand the MWCEC controls access to personal information in order to protect the privacy rights of our volunteers. And I understand that my personal details may be released to other relevant organisations such as the Department of Environment and Science (DES) or Veterinary surgeons to help achieve the objectives of the MWCEC.
3. Agree to adhere to and abide by:
The MWCEC core values, guidelines and Rehabilitation Permit for wildlife caring.
4. Rules set out in the following legislation: Queensland Nature Conservation Act 1992; Code of Practice – Care and rehabilitation of orphaned, sick or injured protected animals by wildlife care volunteers (Nature Conservation Act 1992); Animal care and Protection Act (2001)
5. Understand that the MWCEC or BOOBOOK (founders of MWCEC) accepts no liability for any personal injury, property damage or financial cost arising from any volunteer work undertaken for or on behalf of the group.
6. Acknowledge and understand that as a volunteer of MWCEC, I am not covered by workers’ compensation or any other insurance policy through MWCEC.
7. Fully understand that as part of my volunteer work for MWCEC, I may come into contact with animals either by directly handling them, housing them, or through assisting with their care.
8. Release MWCEC, its Management Committee and BOOBOOK (founders of MWCEC) and their Directors from any loss, damage, injury or expense that I, my family and/or guests may suffer as a result of my becoming a Volunteer Wildlife Carer.
9. Understand that I may be visited by Queensland Department of Environment and Science representative to ensure my facilities are sufficient for wildlife caring.
10. Understand that as a Wildlife Carer, I must:
- Complete the MWCEC Volunteer Wildlife Carer Induction prior to accepting any animals;
- Submit the signed MWCEC Volunteer Wildlife Carer Waiver and Release of Liability Form prior to accepting any animals
- Complete for each animal retrieved or received the “MWCEC Injured Wildlife Initial Record Form” and send to MWCEC administration
- Submit a scanned copy of my volunteer Blue Card to MWCEC

Declaration - Wildlife Carer Only (please read and accept by checking the box and clicking next)

As a potential member who cares for (or intends to care for) wildlife I confirm that before taking and/or caring for wildlife under the MWCEC permit I :

1. Am an approved volunteer member of the Maranoa Wildlife Caring & Education Centre (MWCEC) and I will be operating under the MWCEC Rehabilitation Permit for wildlife caring.
2. Am medically fit and up to date with all vaccinations required and recommended before coming into contact with wildlife, including Tetanus (compulsory); Lyssavirus (compulsory if handling any bats) and QFever (optional, however recommended).
3. Acknowledge and understand that I will comply with the MWCEC Volunteer Code of Conduct and all MWCEC policies and procedures produced and updated by the organisation from time to time;
4. Acknowledge and understand that as a volunteer of MWCEC, I am not covered by workers’ compensation or any other insurance policy through MWCEC.
5. Fully understand that as part of my volunteer wildlife caring work for MWCEC, I will come into contact with animals either by directly handling them, housing them, or through assisting with their care.
6. In addition, for Wildlife Carers (members actually caring for and looking after wildlife) I Confirm:
- that I have a good understanding of husbandry and the caring requirements for the wildlife in my care, along with the necessary facilities and resources available to me to ensure I can care for, rehabilitate and keep under control the wildlife in my care that has been passed onto me by BOOBOOK or MWCEC.
- that I have a good understanding and will abide by the rules set out in all relevant legislations and amendments, including but not limited to:
- Queensland Nature Conservation Act 1992
- Code of Practice – Care and Rehabilitation of Orphaned, Sick or Injured Protected Animals by Wildlife Care Volunteers
- Animal Care and Protection Act
7. Understand that working with animals carries a risk of injury and it is possible that I, my family and/or guests may be bitten, scratched and/or otherwise injured. I freely accept and fully assume all risks, dangers, hazards and liabilities to myself, my family and/or guests, in becoming a Volunteer Wildlife Carer for MWCEC.
8. Release MWCEC and BOOBOOK from any loss, damage, injury or expense that I, my family and/or guests may suffer as a result of my becoming a Volunteer Wildlife Carer.
9. My acknowledgement to this Waiver and Liability Release of my own free will attests to my intent to hold harmless and release from all liability MWCEC and BOOBOOK and any of their past, present or future officers, agents, volunteers or employees to the maximum extent possible under applicable law.
